What you will do
• Physiotherapy Assessment: Undertake comprehensive physiotherapy assessments for older adult inpatients requiring rehabilitation.
• Rehabilitation Planning: Develop and implement evidence-based treatment plans to support recovery and functional independence.
• Caseload Management: Independently manage and prioritise a varied caseload on a designated ward.
• Treatment Delivery: Deliver high-quality physiotherapy interventions tailored to individual patient needs and goals.
• Multidisciplinary Working: Work collaboratively with Occupational Therapists, Nurses, Doctors, and other healthcare professionals to provide integrated care.
• Goal Setting: Establish realistic rehabilitation goals and regularly review patient progress.
• Discharge Planning: Contribute to safe and effective discharge planning to support positive outcomes and continuity of care.
• Service Development: Participate in team meetings and contribute to ongoing service improvement initiatives.
• Clinical Documentation: Maintain accurate, timely, and professional patient records in accordance with departmental and professional standards.
