OCCUPATIONAL THERAPIST - COMMUNITY REHAB Your World is currently seeking an experienced band 6 Community Rehab Occupational Therapist for work on a locum basis. You will be visiting patients at home to provide Admission avoidance, Hospital at home, urgent community response, supported discharges, long term condition management with rehab and inpatient rehab on the 15 bedded Coho In Swanage. Assessing for rehab care support, implementing day to day support, reviewing and progressing patients with assistant support. Managing own caseload and supporting junior staff day to day. You will be joining an outstanding set-up within a well-known Community Rehabilitation Team in Wareham.
