<h3>What You Will Do</h3>

<ul>
<li>
Manage a dedicated caseload focused on ASC, including home assessments and specialist equipment provision
</li>

<li>
Conduct manual handling and complex seating assessments, ensuring effective care planning
</li>

<li>
Complete housing adaptation specifications such as wet rooms and stairlifts
</li>

<li>
Deliver reablement and care package reviews, including training for carers
</li>

<li>
Support duty functions including equipment ordering and hospital discharge facilitation
</li>

<li>
Make welfare calls and carry out home visits when required
</li>

<li>
Work collaboratively with key partner services such as NRS, IIT, ACRT, CMHT and community rehab teams
</li>

<li>
Participate in regular business and development meetings
</li>

<li>
Engage in project activity including telecare assessments in partnership with Camden Careline
</li>

<li>
Complete housing assessments and maintain accurate documentation using Mosaic and other software systems
</li>

<li>
Travel for out-of-borough assessments as required
</li>
</ul>
