What you will do:

<ul>
<li>
Independently manage and prioritise a caseload across multiple Leeds community hubs.
</li>

<li>
Provide expert care and assessments for adults with varied needs, including frailty, falls, dementia, MSK, and neurological conditions (e.g. Parkinson's, MS).
</li>

<li>
Apply the Mental Capacity Act when supporting clients in decision-making processes.
</li>

<li>
Recommend and provide basic equipment, aids, and adaptations to support independent living.
</li>

<li>
Travel across designated areas-car driver with business insurance is essential for this role.
</li>

Requirements:

HCPC registered Occupational Therapist
</li>

Minimum 2 years post-registration experience
