What you will do:

Undertake holistic occupational therapy assessments within home and community settings, identifying functional needs to promote independence

Recommend and arrange equipment, assistive technology, and major/minor adaptations, including Disabled Facilities Grant referrals where necessary

Complete moving and handling assessments, providing crucial guidance and risk management plans for carers and support providers

Develop person-centred support plans in line with the Care Act 2014, focusing on prevention, reablement, and least restrictive interventions

Collaborate effectively with social workers, housing teams, health professionals, and external providers to ensure integrated care and smooth discharge planning

Qualifications:

Degree or Diploma in Occupational Therapy

Evidence of continuing professional development relevant to adult social care practice

Registration and Compliance:

Registered with the Health and Care Professions Council HCPC

Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding adults procedures

Understanding of Disabled Facilities Grants, moving and handling legislation, and strengths-based practice frameworks

Experience:

Demonstrated experience working as an Occupational Therapist in adult social care, rehabilitation, or community services

Proven track record of undertaking functional assessments, moving and handling assessments, and adaptation recommendations

Experience supporting adults with complex physical, cognitive, or sensory needs
-
Competence in working within safeguarding and multi-agency frameworks

Previous experience managing a caseload independently within a local authority or NHS setting
