This full-time opportunity allows you to work from Monday to Friday, giving you the chance to make a real difference in an acute hospital setting. If you have a knack for assessment, treatment planning, implementation, and discharge planning/facilitation for general hospital patients, then we want to hear from you! We are open to considering Band 5 candidates depending on their experience, especially those familiar with HCOP and General Medicine, as well as areas like admission avoidance, Trauma &amp; Orthopaedics, Stroke, and Rehab.
