<h3>What You Will Do</h3>

Complete specialist and highly complex occupational therapy assessments in patients' homes, clinics, and wider community settings.
</li>

<li>
Develop, deliver, and evaluate tailored rehabilitation and treatment plans.
</li>

<li>
Provide expert guidance on complex conditions including neurological diagnoses, frailty, orthopaedics, mental health needs, long-term conditions, and palliative care.
</li>

<li>
Lead on moving and handling strategies, equipment provision, and home adaptations.
</li>

<li>
Support safe, timely hospital discharge through community-based follow-up and liaison with acute teams.
</li>

<li>
Manage a caseload of complex patients, prioritising clinical need and service capacity.
</li>

<li>
Work collaboratively with physiotherapists, social workers, nurses, GPs, and voluntary sector partners.
</li>

<li>
Participate in multidisciplinary meetings and carry out joint assessments.
</li>

Identify, assess, and escalate safeguarding concerns appropriately.
