What you will do
- Carry out comprehensive assessments for individuals with a range of mental health needs.
- Develop and deliver person-centred treatment plans in partnership with service users and the multidisciplinary team.
- Complete risk assessments and implement effective risk management strategies.
- Provide therapeutic interventions, support, education, and guidance to service users and their families.
- Use clinical assessment tools and outcome measures to monitor progress and inform treatment.
- Ensure all interventions align with local and national guidance, including NICE recommendations.
- Contribute to care planning and maintain accurate, up-to-date clinical records in accordance with legal, ethical, and professional standards, including the Mental Health Act.
- Identify safeguarding concerns and take appropriate action to protect vulnerable individuals.
- Travel independently across different locations to deliver community-based services where required.
