What you will do:

• Provide immediate assessment and intervention for patients, ensuring timely and effective care.
• Collaborate with multidisciplinary teams to develop and implement personalised care plans.
• Conduct home assessments to support safe and effective hospital discharges.
• Support patients and their families by explaining treatment plans and signposting appropriate services and resources.
• Maintain accurate and comprehensive patient records in line with professional standards.

Qualifications and Requirements:

• Degree or recognised qualification in Occupational Therapy.
• Current HCPC registration as an Occupational Therapist.
• Experience working within adult social care, community services, or discharge planning.
• Excellent assessment, communication, and clinical decision-making skills.
• Ability to work independently and effectively within a multidisciplinary team.
