What you'll\nbe doing

You'll be responsible for delivering safe,\neffective and compassionate physiotherapy care as part of an integrated\nclinical service. Day-to-day, you will:
- Carry out comprehensive, holistic assessments
- Determine suitability for home or care-home-based physiotherapy\n support.
- Create and implement personalised physiotherapy treatment plans
- Complete medical observations in line with clinical pathways
- Provide patient and carer education on mobility and functional\n equipment to support independence and prevent injury.
- Deliver and monitor exercise treatment programmes, assessing\n progress and adapting plans accordingly.
- Attend case conferences and multidisciplinary meetings,\n contributing clinical insight and supporting coordinated care.
- Communicate effectively with patients, families, colleagues, and\n external partners to ensure safe and efficient care delivery.

Who we're\nlooking for

You'll be a qualified and compassionate\nclinician who thrives in a fast-paced, community-focused environment. You will\nbring:
- Degree-level qualification (or equivalent) in Physiotherapy.
- Active HCPC registration.
- Experience working in an acute setting, having completed\n rotations such as respiratory, orthopaedics, surgery, and care of the\n elderly.
- Experience working within a multi-disciplinary team.
- A track record of contributing to rapid response services is\n advantageous.
- Ability to work autonomously while being a collaborative team\n member.
- Strong professional standards aligned with HCPC code of conduct.
- Confident IT skills (email, Excel, clinical systems).

This role requires regional travel so you must hols a full UK licence
