We're looking for a Clinical Case Manager to join our Case Management team, supporting safe, effective and timely patient care across HCA Healthcare UK, Based at the Harborne Hospital in Birmingham.

The Harborne Hospital provides comprehensive medical and surgical care across major specialties - including orthopaedics, cardiology, gastroenterology, cancer care and urgent care - all within a modern, purpose-built facility designed for seamless patient pathways. Located on the Queen Elizabeth Hospital Birmingham campus, The Harborne Hospital is part of a unique joint venture partnership with University Hospitals Birmingham, combining cutting edge private care with deep clinical infrastructure and specialist expertise.

Our Case Management team plays a vital role in shaping the patient journey, coordinating clinical pathways, reducing delays, and ensuring our patients receive high-quality care alongside full visibility of their financial coverage.

As a Clinical Case Manager, you'll work closely with patients, consultants, multidisciplinary teams, insurers, embassies and internal departments. You'll assess clinical needs, coordinate discharge planning, optimise resource utilisation, and help ensure treatment is appropriately authorised and reimbursed. This role has a direct impact on the overall patient experience - reducing anxiety, enabling smoother transitions, and supporting improved outcomes.

What you'll do:
  • Complete utilisation reviews for patients, ensuring the right level of care and identifying needs early in the pathway.
  • Coordinate, participate in and facilitate MDT meetings to align discharge planning and patient care plan requirements.
  • Act as a key liaison between patients, consultants, insurers, embassies and external agencies.
  • Support complete discharge planning from admission through to safe, timely discharge, including equipment needs and community referrals.
  • Track and trend denials to identify root causes and support resolution.
  • Maintain thorough, accurate documentation in line with internal and external standards.
  • Promote effective resource utilisation and act as an advocate for patient needs.
  • Uphold safe and compliant practice in accordance with the UK Data Protection Act and HCA policies.
  • Contribute to departmental KPIs, quality measures and service improvements.
What you'll bring:
  • NM registered Adult Nurse (RGN) with a minimum of five years' experience in an acute hospital setting OR extensive experience within healthcare insurance.
  • Experience as a Senior Nurse/ band 6 with extensive bedside patient experience (Essential) . Ideally you will have some paediatric nursing experience also (Desirable)
  • Knowledge of inpatient case management processes, discharge planning and community resources essential
  • Strong experience interacting with senior clinical and operational teams.
  • A proactive, dynamic problem-solving approach with the ability to manage multiple priorities.
  • Excellent communication, documentation and interpersonal skills.
  • Confidence in challenging undesirable outcomes and escalating appropriately.
  • Ability to work under pressure and manage emotionally sensitive situations,
  • Strong teamwork ethos with the flexibility to support across sites where required. The ability to take initiative to work independently and within teams
  • Possess excellent communication, organisation and computer skills and have a dynamic approach to problem solving.
  • Strong stakeholder engagement skills at operational and leadership level.
