What you will do

  • Deliver high-quality community-based physiotherapy interventions within patients' homes.
  • Work collaboratively within an integrated health and social care service alongside Occupational Therapists and support staff.
  • Apply your rehabilitation and orthopaedic experience to support patient recovery and maximise independence.
  • Assess, recommend, and provide minor equipment to support daily living and mobility needs.
  • Conduct holistic assessments to identify and address wider health and wellbeing needs.
  • Complete falls risk assessments and implement appropriate prevention and management plans.
  • Develop and review personalised treatment plans to achieve positive patient outcomes.
  • Maintain accurate clinical records and documentation in line with professional standards.
  • Promote independence, functional mobility, and quality of life through evidence-based interventions.

Requirements

  • HCPC Registration: Current registration as a Physiotherapist with the Health and Care Professions Council.
  • Driving Licence: Full UK driving licence essential for community visits

Salary range

  • £26 - £30 per hour
