Carry out initial therapy and functional assessments for patients on the acute medical ward.</li><li>
Plan and support safe, timely discharges from hospital to community settings.</li><li>
Make appropriate onward referrals, including to community hospital beds and additional therapy services.</li><li>
Work collaboratively within a multidisciplinary team to ensure coordinated patient care.</li><li>
Effectively prioritise and manage your own caseload, working autonomously where required.</li><li>
Escalate complex clinical issues to the MDT promptly and appropriately.</li></ul>
Requirements:<ul>

Registration with the HCPC is essential.</li><li>
A minimum of 2 years’ experience in an acute medical physiotherapy setting.</li><li>
A valid driving licence is preferable to support community-related work.</li></ul>
