What you will do
• Assessment and Rehabilitation: Undertake comprehensive occupational therapy assessments and deliver rehabilitation interventions for patients with complex needs.
• Hospital Discharge Planning: Lead and contribute to effective discharge planning, ensuring safe transitions from hospital to home or alternative settings.
• Caseload Management: Manage a specialist caseload within acute and physical health environments, prioritising workloads effectively.
