What you will do

Referral Management: Screen and prioritise incoming referrals, making timely decisions based on urgency, risk, and need.
Urgent Interventions: Support urgent cases, including hospital discharge facilitation and admission prevention initiatives.
Assessment: Conduct comprehensive occupational therapy assessments focused on independence, mobility, and activities of daily living.
Equipment Provision: Recommend and arrange community equipment and assistive technology to maximise independence and safety.
Adaptations Assessment: Assess for minor and major adaptations and facilitate appropriate referrals where required.
Multi-Agency Working: Work collaboratively with service users, carers, healthcare professionals, and partner organisations.
Care Planning: Develop practical solutions and interventions to support individuals in achieving positive outcomes.
Record Keeping: Maintain accurate and timely documentation in line with the Care Act 2014 and local authority requirements.
