This exciting opportunity offers a competitive salary of £28.40 hourly. You will work Monday to Friday from 8.45 to 17.15, totaling 37 hours a week. What you will do:

Assess the needs of service users and develop outcome-focused, person-centred occupational therapy plans
Co-produce care and support plans where necessary to ensure holistic care
Provide expert advice on services, equipment, and support options available to promote independence and wellbeing
Deliver and oversee reablement programs aimed at maintaining independence and enhancing quality of life
Prescribe and arrange necessary equipment and adaptations to meet the specific needs of individuals
Regularly review and reassess the support provided to ensure its effectiveness and make improvements as needed
Collaborate with other professionals and agencies to provide comprehensive support
Offer guidance and support to service users, their families, and carers
Maintain accurate case records and utilise IT systems for efficient case management
Participate in duty, team development, and performance tasks to contribute to the community’s well-being
Maintain HCPC registration and adhere to set policies and standards

Requirements:

Enhanced DBS (Adults and Children's Barred List) and DFE form required – all submissions without these will be rejected
Candidates must be experienced in manual handling and optimal handed care, ensuring a high standard of occupational therapy
