The role is hybrid and the office is based in Gloucester at the GL1 Leisure Centre, so you need to drive and have your own transport. We support Families and young people so we are looking for candidates with any experience of working with children/young people . The role will also involve working at least 2-3 evenings a week with the latest delivery being until approx 19:30. The Health and Wellbeing Coach provides personalised support to individuals to empower them take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of: <ul><li> Healthy Eating and Nutrition </li><li> Smoking Cessation </li><li> Physical Activity </li><li> Weight Management </li><li> Alcohol consumption </li><li> NHS Health Checks (outreach) </li></ul>As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life. The Health and Wellbeing Coach is also responsible for delivering face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one-to-one and group-based support in line with need, location and offer. To provide a health coaching service for eligible participants that supports people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self-identified health and wellbeing goals, including, but not limited to: <ul><li> Triage and assessment </li><li> Agenda setting </li><li> Lifestyle behaviour change support to (e.g.) stop smoking, move more, achieve a healthy weight/diet, reduce alcohol consumption levels. </li><li> Problem solving </li><li> Motivational interviewing </li><li> Goal setting </li><li> Signposting </li></ul>Delivery multi-channel, personalised health coaching support to individuals and groups in line with their needs and preferences including: <ul><li> Face-to-face, telephonic, and digital health coaching </li><li> Multi-mix communications methods including in-app messaging, text, and email. </li><li> Share helpful and supportive resources in line with goal, stage of journey, confidence, and motivation. </li><li> Advocate the use of behaviour tracking tools (online or offline) </li><li> Appraise individuals and population caseloads daily to ensure participants are provided with the right intensity and frequency of support to drive sustained behaviour change. </li><li> A strength-based approach, that maximising opportunities to utilise local community assets and efficient utilisation of the wider health and wellbeing economy. </li></ul>To deliver services in line with quality standards, national / government guidelines, and role boundaries, with the ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately - inclusive on supporting onward referral to wider services. Support with outreach community engagement work inclusive, but not limited to: <ul><li> Screening services (e.g., NHS Health Checks) </li><li> Community and stakeholder events </li><li> External meetings and networks </li><li> Roadshows and pop-ups </li></ul> To deliver services in line with quality standards, national / government guidelines and role boundaries, with the ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately - inclusive on supporting onward referral to wider services.
