What you will do:
- Conduct comprehensive assessments of patients recently discharged from hospital settings.
- Develop and implement tailored rehabilitation plans to meet individual patient needs.
- Collaborate with multidisciplinary teams to ensure seamless patient care and transition.
- Provide training and support to junior staff and healthcare assistants.
Qualifications / Requirements:
- Must be registered with the HCPC.
- Possess a minimum of 2 years of experience in a related specialism.
- A car driver is preferable to facilitate visits across locations.
