Role overview: 
 This specialist Band 6 role sits within the Urgent Community Response (UCR) and Discharge to Assess (AADS) Service. You will work across hospital and community settings, supporting early discharge, admission avoidance, and rapid response services, including Falls Pick-Up, Intermediate Care, and Emergency Department admission prevention. The service operates closely with social care partners, including Reablement services and hospital social workers, ensuring seamless transitions across care pathways.

What you will do:

Assess and treat patients using evidence-based physiotherapy interventions in acute and community settings

Facilitate early hospital discharge and prevent unnecessary admissions
</li>

Work collaboratively within a multidisciplinary team to deliver urgent community responses
