An exciting opportunity for a Therapy Assistant Practitioner has arisen in the Sudbury Community Health Team. The team are based at the Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way.
The successful applicant will join a friendly team of Community Nurses and Therapists. Following training, as required, to use skills from the professional areas of Occupational Therapy and Physiotherapy, you will be providing therapy support to patients in the community. You will be visiting the patients by carrying out home visits and implementing the patient’s individualised care plan designed by the therapists. Under the direction of the therapy staff you will also be taking responsibility for assessing, treating and managing patients with particular emphasis on those patients who have had a fall by following a clear falls intervention pathway.

- prepare and carry out routine elements of patient assessment
- plan and take responsible for care/treatment
- monitor patients and alert a registered practitioner when there are unexpected changes
- modify approaches and activities within care/treatment using own clinical reasoning, notifying a registered practitioner accordingly
- demonstrate problem solving, contributing to solution, working with colleagues
- contribute to discharge planning and make referrals

work with all healthcare professionals and statutory and non-statutory agencies to provide a seamless, integrated service.
