Exciting Opportunity: Band 7 Pharmacist in Specialist Older Persons Medicine in Portsmouth, Hampshire. Earn up to £34.98 per hour hourly with this wonderful ongoing locum position. Work full-time hours at Queen Alexandra Hospital as part of the Older Persons Medicine team. This unique role includes involvement in a Non-Medical Prescribing position within OSDEC and Medical Village (AMU wards), offering room for personal and professional growth in a dynamic environment.

What you will do:
- Lead the Older Persons Medicine pharmacy service at Queen Alexandra Hospital.
- Provide specialist clinical pharmacy support to frailty and elderly care patients.
- Support the development and delivery of pharmacy services within frailty pathways.
- Collaborate with multidisciplinary teams to enhance patient care pathways.
- Develop integrated pharmacy services bridging primary and secondary care.
- Offer expert advice on frailty medicine and polypharmacy.
- Lead and support structured deprescribing initiatives on frailty and elderly wards.
- Deliver expert clinical pharmacy services to various wards.
- Participate in dispensing and carry out pharmaceutical duties.
- Support the development of integrated neighbourhood teams and primary care links.
- Tutor, mentor, and develop junior pharmacists.
- Engage in service development and pathway redesign projects.
- Contribute to education and training programmes within the department.

Requirements:
- A Master's or Bachelor's Degree in Pharmacy, with registration from the General Pharmaceutical Council.
- A Certificate in Clinical Pharmacy, with a Diploma in Clinical Pharmacy completed or in progress.
- Non-Medical Prescribing qualification or willingness to work towards it.
- Relevant hospital pharmacy experience, particularly in multidisciplinary settings.
- Strong communication and interpersonal skills.
- Ability to manage and prioritise workload effectively, motivating yourself and others.

Desirable Requirements:
- Membership of the Royal Pharmaceutical Society or a specialist pharmacy practice group.
- Experience in frailty or Older Persons Medicine pharmacy services.
- Experience leading service development initiatives and participating in multidisciplinary teams.
