What you will do:

• Manage a diverse caseload of residents with complex needs, employing your skills in assessment and intervention.

• Use your strong moving and handling skills to conduct assessments for both major and minor adaptations, enhancing client mobility and accessibility.

• Engage in three days of office visits, connecting with clients on a personal level and understanding their unique circumstances.

• Apply your knowledge and experience from social services settings or the community, such as rapid response environments.

• Meet CPD requirements and maintain your HCPC registration to deliver quality care.

• Drive to various locations as needed, taking advantage of the hybrid setup to meet clients wherever support is needed.
