About the Role
An exciting opportunity has arisen for a motivated and experienced Reablement Occupational Therapy Assistant to join the dynamic Reablement Service in Hackney. This role offers the chance to make a meaningful contribution to residents' independence and wellbeing, supporting people to regain confidence and essential daily living skills following illness, injury, or changes in circumstance.
Working as part of a collaborative multi-disciplinary team, you will assist qualified Occupational Therapists in delivering high-quality reablement interventions and community-based support across Hackney. This is a rewarding position that combines direct client work, problem-solving, and creativity within a supportive environment.
Key Responsibilities

Work in partnership with Occupational Therapists to deliver strengths-based assessments and interventions for individuals in need of reablement support.

Carry out visits to clients' homes to review progress, implement therapy programmes, and promote independence in daily living activities.

Support with the prescription, fitting, and safe use of equipment and minor adaptations, ensuring compliance with safety and quality standards.

Provide advice, retraining and practical support to residents and carers, enabling individuals to achieve identified outcomes.

Assist in the preparation of specifications for minor and major adaptations in consultation with an Occupational Therapist.

Conduct mobility assessments to support applications for concessionary travel schemes such as Blue Badges, Freedom Passes, and Parking Bays.

Maintain accurate, up-to-date case records and contribute to service monitoring and evaluation.

Adhere to relevant legislation, safeguarding procedures, and organisational policies to ensure consistent, high-quality service delivery.

Essential Criteria:

Experience working in a reablement, rehabilitation, or health and social care setting.

Understanding of occupational therapy principles and the reablement approach.

Excellent communication, organisational and record-keeping skills.

Ability to work both independently and as part of a multidisciplinary team.
