What you will do:

• Provide appropriate support at home to prevent hospital admission for service users.
• Work to reduce the need for escalation of care to non-home settings.
• Facilitate timely returns to usual places of residence following temporary escalations of care.
• Collaborate effectively within a multi-disciplinary team to meet the hospital discharge operating model requirements.
• Conduct targeted work to enhance independence through Reablement services.
• Perform Occupational Therapy specific assessments and interventions to maximise independence and help individuals meet their personal goals.
• Provide necessary equipment, minor adaptations, and assistive technology prescriptions.
• Review and adapt Reablement plans, providing advice to non-qualified staff, students, and Reablement colleagues.
• Promote a Reablement ethos throughout the service.

Requirements:

• HCPC Registration: Must be HCPC registered.
• Driving Requirement: Must be a confident car driver to navigate Islington effectively.
