Detailed job description and main responsibilities
To be part of an integrated health and social care team. To provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge.
To provide assessments, and treatment, where required for vulnerable patients with potentially complex needs. To support patient-led goal working within the community.

- Are you Registered with Health Professions Council?
- What post qualification course have you completed that are relevant to this role?
- Have you completed Educator Training or have you supported a student?
- What experience do you have of providing supervision to students and/or colleagues?
