What you will do:
- Assess the needs of clients who are ill or have disabilities for equipment and adaptations, guiding them towards greater independence.
- Provide tailored information and advice to clients, ensuring they are well informed about their options.
- Conduct assessments for clients being discharged from hospitals or care facilities, determining their equipment needs for a successful transition home.
- Collaborate with agencies such as charities, NHS, and other social services to secure additional funding when necessary.
- Work closely with architects, housing departments, and clients to ensure the successful planning and implementation of adaptations.
- Engage with various agencies to maximise information sharing and improve client outcomes.
- Negotiate and make budget-conscious decisions on the most suitable adaptations for clients, ensuring efficient use of resources.
- Ensure all adaptations meet the assessed needs and provide any additional equipment required for optimal use.
- Stay updated with developments in the Occupational Therapy field and arrange for equipment evaluations if necessary.
- Maintain knowledge of moving and handling techniques to better support care staff and clients.
- Evaluate clients for rehabilitation potential and refer them to the appropriate service when appropriate.
