What you will do:

  • Undertake comprehensive occupational therapy assessments for adults following hospital discharge or during periods of crisis.

  • Deliver strengths-based interventions that support independence, recovery, and wellbeing.

  • Develop, implement, and review person-centred rehabilitation plans tailored to individual needs and goals.

  • Support individuals in regaining essential daily living skills, confidence, and control over their lives.

  • Facilitate safe, timely, and effective hospital discharges in collaboration with health and social care professionals.
