What you will do:
- Collaborate on reablement cases with Reablement Officers, Social Workers, and health professionals
- Conduct Occupational Therapy assessments tailored for reablement services
- Complete Double Handed Care reviews and moving and handling risk assessments
- Plan and specify straight-forward home adaptations such as wet rooms and stairlifts
- Use a strengths-based and person-centred approach to boost independence and wellbeing
- Prescribe and promote the use of specialist equipment and adaptations
- Aim to reduce or delay the need for formal care, residential placements, and hospital admissions
- Support Reablement Officers in developing SMART goals for clients
- Undertake joint visits with Reablement Officers and Social Workers to support clients' rehabilitation
- Demonstrate rehabilitation techniques geared towards client autonomy and progress
- Attend team meetings to help manage capacity across the Reablement teams
- Provide professional advice and support to assist the wider Adult Social Care services
- Maintain detailed and accurate records using social care electronic systems
Requirements:
- Degree in Occupational Therapy and HCPC Registration
- Experience in Adult Social Care and completing Occupational Therapy assessments
- Proficiency in managing major adaptations and strong moving and handling knowledge
- Ability to work independently and in multidisciplinary teams
- Strong report writing and record keeping skills, particularly electronic
- Experience supporting rehabilitation and independence outcomes
Desirable Requirements:
- Previous experience in Reablement roles and Double Handed Care reviews
- Experience working with Reablement Officers
- Background in Local Authority Adult Social Care
- Expertise in assessing adaptations and equipment
