What you will do

  • Functional Assessment: Support adults with physical disabilities, promoting independence and wellbeing.
  • Risk Management: Identify and reduce risks such as falls, preventing hospital admissions and care escalations.
  • Caseload Management: Manage a complex caseload independently within a supportive team structure.
  • Care Planning: Conduct person-centred assessments and deliver tailored interventions.
  • Equipment & Adaptations: Assess and arrange mobility aids and home adaptations, including grant-related processes.
  • Manual Handling: Complete manual handling assessments and provide specialist advice.
  • Documentation: Maintain accurate, timely records and effectively signpost service users.
  • Multi-Agency Working: Collaborate with service users, carers, and multidisciplinary teams.
  • Safeguarding: Apply safeguarding principles and work in line with the Care Act.
  • Team Contribution: Participate in team meetings and contribute to a positive working environment.

Qualifications / Requirements

  • HCPC Registration: Must be a registered Occupational Therapist.
  • Experience: At least 2 years post-qualification experience, ideally within community social care.
  • DBS Clearance: Enhanced DBS for adults and children is required.
  • Driving Licence: Full UK driving lic

Salary range

  • £27.94 per hour
