What You Will Do:

• Conduct thorough assessments within the Connect team triage service, managing urgent cases efficiently.

• Develop personalised support plans to promote independence and safety for patients.

• Engage in moving and handling activities, ensuring correct techniques and injury prevention.

• Undertake home visits to assess needs, deliver follow-ups, and provide hands-on support.

• Empower clients with practical tools and strategies to enhance daily living and independence.
