Key Responsibilities

• Carry out community-based occupational therapy assessments within a local authority setting.

• Identify needs and recommend appropriate standard and specialist equipment.

• Plan and coordinate minor and major home adaptations to improve safety and accessibility.

• Work closely with service users, families, and partner professionals to develop tailored support plans.

• Ensure accurate, timely, and compliant case recording and documentation.

Requirements

• HCPC registration as an Occupational Therapist.

• UK driving licence and ability to travel within the community.

• Experience in housing adaptations and community assessments is desirable.
