What you will do

• Collaborate with a multidisciplinary team and contribute to holistic patient care within the intermediate care and reablement sectors.
• Provide occupational therapy assessments and interventions across various pathways, including reablement, rapid response, and rehab beds.
• Use your post-graduate experience to manage the complexities of the team's caseload, ensuring high-quality care for all patients.
• Engage with the community and utilise your driving skills to visit patients at home, aiding in their journey to independence.
• Document progress and liaise with other healthcare professionals to ensure the best outcomes for patients.
