What You Will Do:

  • Lead and coordinate tailored rehabilitation programmes alongside reablement officers to support clients in achieving their goals.

  • Manage a varied caseload that includes hospital discharges and community-based referrals, providing up to six weeks of focused rehabilitation.

  • Conduct comprehensive assessments and interventions aimed at promoting independence and preventing long-term care needs.

  • Utilise your clinical expertise in physical rehabilitation and case management within a dynamic, fast-paced community setting.

  • Engage in multi-disciplinary teamwork to ensure holistic, patient-centred care and optimal outcomes.

  • Travel within the community to deliver responsive and effective care directly to clients' homes.

Requirements:

  • HCPC registration as an Occupational Therapist.

  • Minimum of two years' post-qualification experience within adult

  • £39.81 per hour
