Locum Occupational Therapist in Home First Team, Wareham, Dorset, earning up to £35.00 per hour, hourly.

What you will do:

- Support hospital discharges and discharge planning, ensuring seamless transitions for patients
- Deliver reablement-focused Occupational Therapy interventions that promote recovery and independence
- Complete comprehensive Occupational Therapy assessments in community settings
- Collaborate effectively with health and social care partners, building relationships with multidisciplinary agencies and professionals
- Manage a community-based caseload and perform home visits across Dorset
- Contribute to positive health and care outcomes, maintaining accurate records and case documentation

Requirements:

- Degree in Occupational Therapy with HCPC registration
- Minimum 3 years of post-qualified UK Occupational Therapy experience
- Experience in hospital discharge pathways and reablement services
- Knowledge of Occupational Therapy frameworks and practice models
- Understanding of psychological and human development theories
- Ability to work collaboratively with multidisciplinary teams
- Access to a car and a willingness to travel across Dorset
- Enhanced Adult DBS and Right to Work in the UK
- Experience within Home First services and community Occupational Therapy
- Experience working across health and social care systems
