This is an exciting opportunity to join our vibrant and dynamic team in Oakham as an Occupational Therapist working within the Discharge to Assess pathway, on a part-time basis for 21 hours per week, over an initial 3-month contract. What you will do:

  • Provide a range of Occupational Therapy interventions for adults referred to the service, enabling them to remain in their own homes while achieving an optimal level of independence in all aspects of their daily lives.
  • Conduct holistic and strength-based assessments, working collaboratively with individuals to develop personalised goals and reablement plans.
  • Support individuals on a Discharge to Assess pathway, facilitating a smooth transition from hospital to home, and aiding their recovery and independence.
  • Assist individuals living in the community who require therapy reablement to prevent hospital admission.
  • Assess and arrange provision of equipment and adaptations to maximise independence and safety, including major adaptations where necessary.

Requirements:

  • HCPC Registration: Must be HCPC registered.
  • Driving Requirement: Must be a car driver to fulfil this role.
