What you will do:

• Conduct comprehensive assessments of clients in their homes to determine their individual needs and develop personalised care plans.
• Provide expert advice and recommendations to help clients maintain independence in their daily activities.
• Work collaboratively with a multidisciplinary team to ensure the highest standard of care and effective service delivery.
• Travel to clients' homes across West Cornwall, offering support and interventions tailored to specific client needs.
• Maintain up-to-date client records and document progress according to professional and organisational standards.
• Engage in ongoing professional development to stay current in best practices and innovate client solutions.

Qualifications / Requirements:

• Professionally qualified as an Occupational Therapist.
• Registered with the HCPC.
• At least 2-3 years of experience as a Band 6 Occupational Therapist within the physical health or adult social care sectors.
• A valid driving licence and the ability to travel to remote areas for client visits are essential.
