What you will do:

• Provide comprehensive assessments of service users with complex care and therapy needs, collaborating across teams within adult social care.
• Determine and supply equipment that enhances independence in daily living activities.
• Conduct assessments for both major and minor home adaptations, ensuring accurate specifications to promote independence and minimize injury risks.
• Perform manual handling assessments to decrease injury risks for service users and carers, aiming to optimize care packages for single-handed care.

Requirements:

• Must be registered with the Health and Care Professions Council (HCPC).
• A car driver is necessary, so proficiency and comfort with driving are essential for visiting various locations effectively.
