What you will do:

  • Manage your own caseload involving Adult Social Care tasks such as home assessments, recommendations, and equipment provisioning.

  • Conduct manual handling assessments, reviews, and provide specialist seating assessments.

  • Support reablement assessments and reviews to recommend care package adjustments.

  • Prepare specifications and diagrams for major adaptations like wet rooms, stairlifts, and ramping.

  • Provide duty support with equipment management alongside social workers and duty managers.

  • Facilitate welfare contacts through phone and home visits.

  • Collaborate with various health and social care teams including NRS, IIT, ACRT, and CMHT.

  • Attend monthly meetings that focus on business and development.

  • Engage in project work and conduct housing assessments and recommendations.

  • Conduct telecare assessments and collaborate with responders like the Camden care line.

  • Utilize databases and software like Mosaic, Word, and Excel effectively.

  • Be open to out of borough visits as required.

A candidate who is able to adapt to change and willing to take on additional tasks for team support is essential. An enhanced DBS check is required, and you should be a registered driver with a valid HCPC license.
