What You Will Do

  • Undertake comprehensive functional assessments in line with the Care Act, identifying strengths, needs, and desired outcomes.

  • Provide duty triage and screening, reviewing and signposting incoming referrals prior to allocation.

  • Recommend and design major adaptations such as level-access showers, ramps, and stairlifts to support safety and independence.

  • Prescribe suitable equipment and minor adaptations tailored to individual needs, improving quality of life and functional ability.

  • Conduct detailed manual handling assessments and develop safe, person-centred moving and handling plans.

  • Work collaboratively with contractors, housing teams, surveyors, and grants officers to ensure timely, compliant completion of adaptation works.

  • Produce accurate, comprehensive documentation including assessment reports, funding applications, and safeguarding-related records.
