We are delighted to offer an excellent opportunity for a skilled and motivated Occupational Therapist to join the Adult Social Care Reablement Team within Social Service Reablement in Cheshire. This ongoing locum role offers both full-time and part-time flexibility and focuses on supporting adults to regain, maintain, or maximise independence following illness, injury, hospital discharge, or functional decline.

Working within a short-term, goal-focused reablement framework, you will play a vital role in helping people live safely and independently in their own homes, reducing the need for long-term care and delivering meaningful, outcomes-driven support.

What you will do:

• Carry out occupational therapy assessments to identify functional abilities, risks, and reablement potential.

• Deliver short-term, outcome-focused interventions to promote independence in daily living activities.

• Develop, implement, and review personalised reablement plans with clear goals and timescales.

• Recommend, prescribe, and review equipment, assistive technology, and minor adaptations.

• Contribute to hospital discharge planning to ensure safe and timely transitions into the community.

• Work collaboratively with reablement support workers, social workers, healthcare professionals, and partner agencies.

• Apply relevant Adult Social Care legislation, including the Care Act 2014 and Mental Capacity Act 2005.

• Complete risk assessments and manage risk using sound professional judgement.

• Maintain accurate, compliant records, assessments, and reports.
