We are delighted to offer an excellent opportunity for a skilled and motivated Occupational Therapist to join the Adult Social Care Reablement Team within Social Service Reablement in Cheshire. This ongoing locum role offers both full-time and part-time flexibility and focuses on supporting adults to regain, maintain, or maximise independence following illness, injury, hospital discharge, or functional decline.
Working within a short-term, goal-focused reablement framework, you will play a vital role in helping people live safely and independently in their own homes, reducing the need for long-term care and delivering meaningful, outcomes-driven support.

What you will do:

Carry out occupational therapy assessments to identify functional abilities, risks, and reablement potential.

Deliver short-term, outcome-focused interventions to promote independence in daily living activities.

Develop, implement, and review personalised reablement plans with clear goals and timescales.

Recommend, prescribe, and review equipment, assistive technology, and minor adaptations.

Contribute to hospital discharge planning to ensure safe and timely transitions into the community.

Work collaboratively with reablement support workers, social workers, healthcare professionals, and partner agencies.

Apply relevant Adult Social Care legislation, including the Care Act 2014 and Mental Capacity Act 2005.

Complete risk assessments and manage risk using sound professional judgement.

Maintain accurate, compliant records, assessments, and reports.
