What you will do:
- Undertake comprehensive Occupational Therapist assessments for adults aged 18 and over, ensuring they comply with the Care Act.
- Apply expert clinical reasoning skills to assess risks, promote independence, and support safe outcomes for service users.
- Prescribe both simple and complex equipment, completing any necessary risk assessments.
- Conduct Manual Handling assessments and provide appropriate recommendations for moving and handling.
- Evaluate and recommend major home adaptations in alignment with statutory guidelines.
- Participate in the Disabled Facilities Grant (DFG) processes, referring as necessary and providing valuable contributions.
- Engage in duty triaging on a rotational basis, managing referral screenings and responding to urgent cases.
- Ensure accurate, timely, and professional recording of all case notes.
- Collaborate effectively with service users, carers, health professionals, housing teams, and partner agencies to provide comprehensive support.
Qualifications and requirements:
- Must be HCPC registered.
- Must possess a full driving licence to accommodate the community-oriented nature of the work.
