Role overview

Working as part of the Adult Community Services team, you will promote independence, wellbeing, and safe living environments. You will complete strengths-based assessments, recommend equipment and adaptations, and undertake moving and handling assessments to support service users within their homes and communities.

What you will do

  • Conduct comprehensive occupational therapy assessments for adults in community settings.
  • Complete functional assessments to identify needs and risks, ensuring safety and independence.
  • Recommend and arrange minor and major adaptations, equipment, and assistive technology.
  • Undertake moving and handling assessments and provide tailored care recommendations.
  • Work collaboratively with social workers, physiotherapists, housing teams, and health professionals.
  • Promote independence and enablement using person-centred approaches.
  • Complete Mental Capacity Assessments and contribute to Best Interest decisions where required.
  • Maintain accurate and timely case records in line with legislation and organisational policies.
  • Participate in safeguarding enquiries and risk management processes when necessary.
  • Effectively manage a diverse and complex caseload, prioritising workload appropriately.
