Southend Integrated Healthcare for the Homeless (SIHH) is seeking an enthusiastic and compassionate Prescribing Paramedic or Nurse to join our dedicated team, providing specialist healthcare services to people experiencing homelessness across Southend-on-Sea.

About the role:
As part of our innovative homeless healthcare service, you will deliver primary care and outreach clinics across a range of community settings, working closely with local charities, homeless support services and voluntary organisations.
You will play a key role in identifying unmet health needs, supporting patients to access wider healthcare services, and helping to reduce health inequalities within the local homeless population.
The role is supported by an experienced multidisciplinary team, including a GP Clinical Lead, Practice Nurse and other healthcare professionals during selected sessions.
Previous experience working with homeless or marginalised groups would be advantageous, but we welcome applications from clinicians with a genuine interest in this field and a commitment to improving outcomes for vulnerable populations.
What you'll be doing:
- Deliver specialist primary care consultations and outreach clinics within community settings, including local soup kitchens and homeless support organisations.
- General physical health reviews
- Health screening and prevention
- Wound assessment and management
- Lifestyle and wellbeing assessments
- Promote health and wellbeing through proactive interventions and health education
- Primary care services
- Secondary care providers
- Mental health services
- Drug and alcohol services
- Community and voluntary sector organisations
- Develop and maintain effective pathways into specialist services to ensure patients receive the right care at the right time
- Act as a key point of contact between healthcare providers and community services
- Deliver regular drop-in clinics across various homeless support locations in Southend
- Champion preventative healthcare, including promoting female health screening and wider public health initiatives
About the Candidate

- A qualified Prescribing Paramedic or Registered Nurse Prescriber
