What you will do

• Deliver expert clinical pharmacy services to medical and surgical inpatients.

• Undertake medicines reconciliation, clinical screening, and comprehensive medication reviews.

• Work collaboratively with doctors, nurses, and allied health professionals to optimise patient care.

• Identify, prevent, and resolve medication-related issues to improve safety and outcomes.

• Provide pharmaceutical advice and support to ward-based teams.

• Contribute to medicines optimisation initiatives and service improvements.

• Support and supervise junior pharmacy staff and trainees where appropriate.

• Maintain accurate clinical documentation in line with local policies and regulatory standards.
