What you will do:

Provide urgent physiotherapy assessments and interventions to support hospital admission avoidance.

Respond promptly to referrals for patients experiencing acute mobility, functional, or rehabilitation needs.

Undertake comprehensive assessments of mobility, balance, strength, transfers, and falls risk within community settings.

Deliver timely rehabilitation programmes aimed at maximising independence and preventing further deterioration.

Develop and implement person-centred treatment plans to ensure patients can remain safe at home.

Work collaboratively with multidisciplinary teams to provide coordinated crisis response services.

Support early supported discharge programmes and contribute to reducing avoidable hospital readmissions.
