You will play a key role in supporting older adults to maintain independence, prevent unnecessary hospital admissions, and promote safe, effective care closer to home. Key responsibilities

• Provide physiotherapy input within the Frailty Team, with a focus on Hospital at Home services.

• Undertake comprehensive assessments and develop tailored rehabilitation and treatment plans.

• Deliver physiotherapy interventions within community settings, supporting older adults with complex needs.

• Work collaboratively with multidisciplinary teams to optimise patient outcomes.

• Contribute to care planning aimed at reducing hospital admissions and supporting early discharge.

• Maintain accurate, timely clinical records in line with HCPC standards and best practice.

Requirements

• HCPC registration as a Physiothera
