This is an exciting ongoing opportunity for a 3-month contract position for a dynamic Band 6 Falls Prevention Therapist (Physiotherapist or Occupational Therapist) in the vibrant community of Ealing, London. The role is full-time with a working schedule of 37.5 hours per week, engaging with a diverse range of patients at risk of falls and frailty in the community. What you will do:

  • Conduct holistic therapy assessments and multifactorial falls risk assessments.
  • Perform comprehensive geriatric assessments for frail patients.
  • Deliver evidence-based falls prevention interventions tailored to individual needs.
  • Manage an independent community caseload, ensuring personalised patient care.
  • Facilitate falls prevention exercise and advice groups to educate and empower patients.
  • Complete onward referrals to specialist services as needed.
  • Monitor outcomes through effective goal setting and outcome measures.
  • Collaborate within multidisciplinary teams to enhance patient care and minimise falls risk.

Qualifications / Requirements / Person Specification:

  • Degree in Physiotherapy or Occupational Therapy.
  • HCPC Registration and previous NHS experience.
  • Strong background in community rehabilitation and managing frailty and falls caseloads.
  • Proven experience in holistic assessments and risk assessments.
  • Robust clinical reasoning skills and ability to prioritise an independent caseload.
