What you will do:

- Provide community-based Physiotherapy assessment and treatment to a diverse caseload.
- Deliver rehabilitation programmes tailored to individual needs in community environments.
- Conduct comprehensive assessments of mobility, function, and rehabilitation requirements.
- Support patients with neurological conditions through expertise and compassion.
- Formulate, implement, and regularly review personalised treatment plans.
- Collaborate effectively with multidisciplinary teams to ensure seamless care.
- Aid in discharge planning and contribute to admission avoidance pathways.
- Keep precise and detailed clinical records and documentation.
- Promote independence and recovery, empowering patients within the community.

Requirements:

- Degree or Diploma in Physiotherapy with HCPC registration.
- Proven Band 6 Physiotherapy experience in community settings.
- Competence in managing an autonomous caseload with strong clinical assessment and reasoning skills.
- Ability to work within multidisciplinary teams and excellent communication skills.
- Organisational capability to support effective and streamlined patient care.
- Experience in neurological and community rehabilitation, and expertise in admission avoidance.
- Familiarity with supporting patients experiencing long-term neurological conditions.
