What you will do

  • Undertake comprehensive physiotherapy assessments for patients with complex physical needs in community environments, including patients' homes, clinics, and care settings.

  • Develop, implement, and review personalised rehabilitation and treatment programmes.

  • Manage a varied caseload including long-term conditions, frailty, mobility issues, post-acute rehabilitation, and falls prevention.

  • Provide specialist physiotherapy interventions and advice to patients, carers, and other healthcare professionals.

  • Support early discharge planning and admission prevention through timely rehabilitation and intervention.

  • Work autonomously, applying advanced clinical reasoning and decision-making skills.

  • Supervise and support Band 5 physiotherapists, assistants, and students, promoting high standards of practice.

  • Contribute to service development, audits, and quality improvement initiatives.
