What you will do:

- Provide expert assessment and intervention for patients with neuro-related issues within the community.
- Develop, implement, and review personalised rehabilitation programmes.
- Liaise with other healthcare professionals for a holistic approach to patient care.
- Conduct home visits and assessments, utilising your own vehicle to reach patients across the Huddersfield and Halifax area.
- Maintain detailed patient records and communicate progress effectively with the wider team.
